Fee rates are available upon request.

Terms

  • Upon scheduling any appointment, a 50% deposit of the fee is required at least 14 days in advance of the session for the booking to be confirmed.
  • The balance of the fee may also be paid in advance or may be paid on arrival.
  • Cancellations less than 7 days before the booked appointment are liable to a £100 charge.

We are glad to accept payment by cheque, cash, BACS transfer or PayPal

Our Forms

There are a number of forms that you will need to complete, prior to the start of the process. We appreciate that this can take a little time, but they will save time when we meet.

All the forms are available in your choice of Word document or Adobe Acrobat pdf, but Word is the best option as the form can be filled on the PC by typing.
Clicking the file links should give you the option to open each file. Otherwise click on your choice of file with the right hand mouse button (or 'Control-Click' on a Mac) and select the option to save the linked file to your own computer. You can then open the file to edit or print it.

1. Initial Self-Referral Form

The purpose of the initial form is simply to ensure we have your contact details and are clear as to which areas you would like mediation to help you with.

This form is best completed by typing into a Word document (download) but if neccessary can be downloaded as a Adobe Acrobat pdf (download).

2. Follow-Up Self-Referral Form

After making contact and your first appointment but before we meet with you, we will ask you to complete the ‘Follow-Up Self-Referral Form’ which goes in to much more detail about the issues you would like to resolve.

This form is best completed by typing into a Word document (download) but if neccessary can be downloaded as a Adobe Acrobat pdf (download).

3. Agreement to Mediate

You will be asked to bring this with you to sign when you attend your first meeting, and a copy of this will then be sent to you.

4. Parenting Plan form

You may also need to complete a Parenting Plan, if appropriate to your circumstances.
